Creating Feedback Questionnaires in Moodle

Create the feedback activity

Go to the Moodle homepage and enable Edit mode in the top-right corner (if not already enabled).

Expand the right sidebar (if not already expanded), then scroll to the bottom of the "Main menu" block and click "Add an activity or resource".

Click "Feedback".

Enter the questionnaire title (Name), description and other details.

In Common module settings > Availability, select "Hide on course page" (until you're ready to publish it).

Optional: In the "Availability" section, set start and end dates.

Click "Save and display".

Populate the form

If you want to use a template: Click "Templates", then select a template to preview it. Once you find the one you want, click "Use this template", then select "Delete old items" and click "Save".

Alternatively, you can create the form from scratch using the controls provided.

Optional: If you need to make any changes to the template, or create a new one, make them on this page, then click "Save as new template".

Assign it to courses

Click "Mapped courses". Select each of the courses from the dropdown, then click "Save changes".

Warning: Do not select the "Mathematical Institute" course (which refers to the homepage) - otherwise we can't tell which course they were trying to submit feedback for.

Make it available to students

When you're happy with it, change Settings > Common module settings > Availability to "Show on course page" to publish it.

To complete the questionnaire, students will need to browse to the relevant course page and click the activity title in the "Feedback" block.

If you would like to send a link directly to students, browse to the relevant course page, click the activity title and copy the URL. Note that it must be in the format https://courses.maths.ox.ac.uk/mod/feedback/view.php?id=123&courseid=456, with the course ID specified, not https://courses.maths.ox.ac.uk/mod/feedback/view.php?id=123 - otherwise students will see the error "You can only access this feedback form from a course". It is not currently possible to share a link that works for all courses.
